The University of South Florida will provide continuing education credits for the Twelfth Annual International Meeting of the American Telemedicine Association, including credit for physicians; nurses; pharmacists; psychologists; and attorney’s; as well as Florida-licensed clinical social workers, marriage and family therapists, and mental health counselors. Attendees who wish to obtain continuing education certificates should complete an Attendance Verification Form, (to be provided with course materials) and return it to the program staff before the conclusion of the meeting. |

Faculty Disclosure
It is the policy of the University of South Florida College of Medicine and the American Telemedicine Association to ensure fair balance, objectivity, and scientific rigor in all presentations. Faculty are expected to disclose any significant relationships with commercial supporters of the annual meeting and/or with manufacturers of any commercial products or services discussed during the educational presentation. Faculty must also disclose any off-label use of products that will be discussed during their presentation. Faculty disclosures are provided for review to all attendees. |

Physicians
This activity has been planned and implemented in accordance with the Essentials Areas and policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint sponsorship of the University of South Florida College of Medicine and the American Telemedicine Association. The University of South Florida College of Medicine is accredited by the ACCME to provide continuing medical education for physicians.
The University of South Florida College of Medicine designates this educational activity for a maximum of 13.25 AMA PRA Category 1 Credits™. Physicians should only claim credit commensurate with the extent of their participation in the activity |

The University of South Florida College of Nursing is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center's Commission on Accreditation. This activity is for 13.25 contact hours. |

Pharmacists
The University of South Florida College of Medicine is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. This program has been approved for 13.25 contact hours. Universal program number is as follows: 230-000-07-004-L04. To receive continuing education credit, a pharmacist must attend the accredited sessions and must return the program evaluation instrument. In order to receive full credit, registrants must register no later than 10 minutes after the start of the meeting and must attend the entire meeting. |

The University of South Florida College of Medicine is approved by the American Psychological Association to sponsor continuing education for psychologists. This activity is offered for up to 13.25 hours of continuing education credit. The University of South Florida College of Medicine maintains responsibility for this program and its content. |

Florida Licensed Clinical Social Workers, Marriage and Family Therapists, Mental Health Counselors
The University of South Florida College of Public Health is an approved provider (BAP# 433. Ex. 3/31/07). This program has been reviewed and approved for up to 16, 50-minute hours. |
